Supported living​

Fortix is a person-centred supported living provider offering accommodation-based support for adults aged 18-65 with complex needs. Our properties are located across Sheffield, Doncaster, Worksop, and Retford, providing a range of housing options to meet individual needs and preferences.
We provide structured, outcome-focused support tailored to each person’s assessed needs, working collaboratively with commissioners, social workers, and partner agencies to promote independence, stability, and positive outcomes.

Who we support

We specialise in supporting adults with complex and overlapping needs. Individuals referred to Fortix often present with multiple vulnerabilities and risks that require a structured, person-centred approach. This includes people with:

  • Mental health conditions including schizophrenia, EUPD, bipolar disorder, PTSD, anxiety, and depression
  • Autism Spectrum Condition, ADHD, and acquired brain injuries
  • Learning disabilities and cognitive impairments
  • Histories of trauma, abuse, neglect, and adverse childhood experiences
    Substance misuse and associated risks
  • Histories of homelessness and accommodation breakdowns
  • Offending or re-offending behaviours

 

We have successfully supported some of the country’s most difficult-to-place individuals, with a strong focus on helping people become stable, manage risk effectively, and move forward at their own pace.

Fortix provides practical, emotional, and outcome-focused support tailored to each person’s assessed needs. This includes:

  • Daily living support and personal care prompts
  • Medication prompts and concordance support
  • Mental health recovery and wellbeing monitoring
  • Positive behaviour support and emotional regulation
  • Budgeting support and financial safeguarding
  • Accommodation sustainment support
  • Community integration and access to meaningful activities
  • Multi-agency working and liaison with commissioners and professionals

Make a referral

To make a referral or discuss a potential placement, please get in touch with our team. We will work with you to assess the individual’s needs and identify the most appropriate support package.
To complete an initial assessment, we request the most recent: Care Act Assessment, Risk Assessments, Support Plans, Mental Health Information, Capacity Assessments where applicable, and any forensic or offending history information where applicable
